Successfully complete all stories with an 'S' rank to unlock the Chaos difficulty setting.

Fifth story

Successfully complete the first four stories with a 'Silver' or higher rank to unlock the fifth story.

Extra item slots

Purchase all the upgrades at the Buy/Ability menu for the stat in the indicated level you are in (for example, attack, defense, life, Musou) to unlock another item slot:

Third slot: Musou: Acendancy
Fourth slot: Musou: Swordsman
Fifth slot: Musou: Stealth
